I want to send my players a short survey to get a feel for how they are doing and what they want to do more of.

Since I don’t want to overload them cognitively with too much free-form feedback, I wanted to ask if there are any simple TTRPG Feedback Survey templates I could use? Preferrably for 5e

(was just curious^^ … I am gaming with them since quite a while now)

I tried to keep it short:

  • what was your favorite adventure/episode of our last campaign and why?

(btw. the results were less useful than I had hoped^^)

also keep in mind that people tend to be more comfy answering positive questions
